Secondary school/high-school diploma, including transcript (a translator-signed translation of the diploma into English or Polish is required if the diploma was issued in a language other than Polish, English, Russian, and Ukrainian). Medical insurance policy or a declaration (available in the Application Portal) of enrollment in Poland's National Health Fund. English proficiency certificate: CEFR B2 (for a list of acceptable certificates please refer to the Documents Section to the right) or mandatory participation in an interview assessing English proficiency. For holders of the Polish secondary school diploma (matura): basic level – minimum 90%, extended level – minimum 60%. Holders of the so-called "polska stara matura" need the final grade 4 or higher on the diploma.
